Introduction
Our office is using Microsoft® HealthVault™ as a tool to provide the patient (you) with the most up to date information regarding your health. In this document, "We" or "Our" refers to our office and Microsoft. Our office is able to send your health information to the Microsoft® HealthVault™. It is important for you to know how the information that is submitted, reviewed and edited in the Microsoft® HealthVault™ and is handled while you use the Microsoft® HealthVault™ site. 
Collection of Personal and Usage Information
Our office will enter data provided by you and the doctor can send that information to Microsoft® HealthVault™ for you to view at anytime. That data is stored in Microsoft® HealthVault™ in your HealthVault account. You will be sent an email with information to allow you access to your HealthVault account to provide you with personalized health information. You can review and edit information from your HealthVault account and any changes will be saved to your HealthVault account. Any changes that you make to your HealthVault account will not be sent to any of your providers.
